CCIC-MSP offers services across several areas (imaging, metabolomics, proteomics, general MS and native MS) in addition to the use of its instruments and equipment to both internal Ohio State users and external users.

Submitting a Service Request
Some information will populate by default and cannot be edited. If there is an issue, please contact fom-admin@osu.edu.
- Enter your billing information.
- Required fields:
- o Is this service related to CCC users?
- CCC is the Comprehensive Cancer Center at OSU
- This box should be checked if PI (or Co-I) is a CCC member.
- o IBC# - this is the Institutional Biosafety Committee ID number
- Investigators must register with the Institutional Biosafety Committee (IBC) if they are:
- creating recombinant or synthetic nucleic acid molecules (vector plus gene)
- inserting recombinant or synthetic nucleic acid molecules into cell lines/tissue cultures, whole animals, humans, or plants
- using or creating transgenic or knock-out animals
- using a microorganism that is pathogenic to humans (including immunocompromised individuals), plants, or animals (based on wild-type organism)
- using or collecting human or non-human primate materials (body fluids, tissues, cells and established cell lines, etc.)

- If you do not need one, put N/A in this field
- Investigators must register with the Institutional Biosafety Committee (IBC) if they are:
- o IRB# - this is the Institutional Review Board ID number
- All activities that meet the definition of both research and human subject must be approved by an IRB before being performed unless the research has been determined to be exempt.

- If you do not need one, put N/A
- If you do have one, note you must upload a copy after you submit this form. See “Submitting Samples” below.
- o Sample Information
- o Note: some additional information may need to be provided after the service request is submitted. See “Submitting Samples” below.

Submitting Samples
Once the service request is created in FOM, as described above:
- Click on “Collaborate & Service” to view your requests.
- Open the associated service request form.
- Upload additional information.
- Print form.
- Bring sample(s) with a printed copy of the service request:
- In-person to our facility, 8am-4pm Mon-Fri
- Mail samples

Editing / Updating a Service Request
Once the service request is created in FOM, as described above, you can edit or update, as needed:
- Click on “Collaborate & Service” to view your requests.
- Open the associated service request form.
- Edit the information necessary.
- “Update This Request”
